PERMISSION TO PHOTOGRAPH:

I grant to Maria Alive, LLC, its representatives and employees the right to take photographs/videos of me and with the Functional Yoga Therapy Training. I authorize Maria Alive, LLC, its assigns and transferees to copyright, use and publish the same in print and/or electronically.

I agree that Maria Alive, LLC may use such photographs/videos of me with or without my name and for any lawful purpose, including for example such purposes as education, publicity, illustration, advertising, and Web content.

I give permission to obtain postural photos from students for the purpose of offering a private session to the student by Maria Shamas, or one of her designated Faculty/Mentors, under the program Functional Yoga Therapy™, which is part of MariaAlive Therapeutics, under MariaAlive,LLC.  

Please note that these photos are to be deleted after the agreed upon session with no further authorization given for other use of the photos, unless explicitly stated and agreed upon by both student and Maria Shamas/Faculty/Mentor.
